Notice of Privacy Practices

Life Alliance shall maintain our consumer’s right to privacy and confidentiality of information at all times. Information known or contained in the consumer’s record (known as protected health information) shall be treated as confidential and will be released only in appropriate circumstances and only with the written consent of the consumer or legal guardian.
Information concerning consumers, families, and staff shall be managed with the highest degree of appropriateness and confidentiality pursuant to organization-wide policies and procedures, federal, state, and local laws, guidelines and regulations, including but not limited to the Health Insurance Portability and Accountability Act (HIPAA).

Below are some of the primary rights afforded to you by HIPAA:

Life Alliance staff are trained on our confidentiality and privacy policies and procedures upon hire and at regular intervals throughout their employment. All potential breaches of confidentiality are reviewed by the Life Alliance Leadership Committee and swift, appropriate action is taken.
